Gentlemen:

\JI am writing you in regard to my policy number 473731037.

We wrote you on May  3rd to inform you that we had traded the '67
Cheverolet in on a 1977 Cheverolet as of April 25.  You were also informed
of this by phone on April 26th and a
request made to increase the coverage on this new car to that provided by
Plan 55.

We were informed on May 3rd by phone that this extended coverage was
already in effect but that we should return the Enrollment form as is,
which we did.

We were told that you would bill us for the additional cost and that it
would not be necessary for us to return the form notifying you of the car
replacement.  We have never received the bill for this additional
coverage.  Will you please let me know how much this is.

I am enclosing a copy of my May 3rd letter for reference.\.
